AZS membership card instructions:
AZS membership cards entitling you to participate in trainings of the section KU AZS Collegium Medicum of the Jagiellonian University and to take part in the Polish Academic Championships, Polish Championships of Medical Universities and Academic Championships of Malopolska and it can be obtained via the website: https://azs.pl/
At the website above after clicking on the appropriate links (on-line registration) you can start to create or extend the AZS membership card. You can choose from 3 types of ID cards:
– ISIC AZS for students,
– ITIC AZS for university employees,
– AZS for other members.
From 01/01/2023 the amount of fees will be increased by PLN 20.
After clicking the appropriate registration link you will be redirected to a simple form that needs to be completed / updated. Please pay attention to the correct choice of club: KU AZS CM UJ Krakow (not KU AZS Jagiellonian University Krakow, or any other)! A mistake made in this place will result in registration in a different club and you will not be able to benefit from co-financing of participation in the competition by KU AZS CM UJ! If you are asked to choose a university, please also choose the Jagiellonian University Collegium Medicum.
You should also select the main section. If the discipline you practice is not on the list of sections and you would like to take part in competitions in this discipline – select the “student” section. This applies in particular to such disciplines as: mountain biking, sport shooting, fencing, horse riding, martial arts and others. The selection of the section does not limit in any way a participation in the activities of other sections it is only important for organizational purposes. Please choose the correct insurance option (the membership fee amount depends on it) and type of the card. The electronic version is sufficient for verification during the competitions. For those willing, it is possible to receive a plastic ID card by mail but for an additional fee.
After the your membership is approved by the Club’s authorities you get an e-mail with information that you can make an online payment. After paying the fee you can consider yourself as a member of KU AZS Collegium Medicum of the Jagiellonian University! You can now download the ID card and insurance certificate from your profile. 
From this year in the “Planeta” system you can also obtain Academic Affiliation Certificates (option available after paying the membership fee). After clicking on the appropriate tab you can fill out a simple form. Remember to select the Jagiellonian University Collegium Medicum in the field of the university (not the Jagiellonian University in Krakow, or any other). If you make a mistake at this point, you will not be able to represent the Jagiellonian University Medical College during the competition! You can download the certificate as a PDF file print it out in at least twice copies. After that please take all copies to the dean’s office where your affiliation will be confirmed by the university employee with the signature and university stamps. Please deliver the stamped and signed certificate to the section coach.
EXCLUSIVELY members of KU AZS CM UJ with paid membership fees for the academic year 2022/2023 (not applicable to the THREE first trainings in October 2022) can participate in training sessions of the sports sections (including Cheerleaders section). Trainings of the recreational sections are open for everyone.
